Submitting a Case Request Through a Public Form

A public case request form lets a requestor provide the information your team needs before deciding whether to start a case.

Complete the intake steps

  1. Open the link supplied by the organization and read its instructions.
  2. Enter the requested company and contact details. Use an email address and phone number where the team can reach you.
  3. Select the case type and available services, then provide the reference number, instructions, and any requested budget information.
  4. Add subject information when the form asks for it. Check names, identifiers, locations, and the primary subject carefully.
  5. Add supporting files when enabled, following the displayed type and size limits.
  6. Review the summary. Use the available Back or edit controls to correct information before the final submission.
  7. Submit and wait for the confirmation page.

After submission

The confirmation page shows Request Submitted Successfully and a request number. Save or copy that number for follow-up. The request is pending review; it is not yet an accepted case.

If you need to correct something afterward

Contact the receiving team with your request number and explain the correction. The public submission confirmation is not a case-management login or a general editor for the submitted record. Avoid submitting the same request again unless the team asks you to, as a second submission can create a duplicate intake record.
