Creating and Configuring Public Intake Forms

Open Settings > Public Forms. This area requires Full Access plus Manage Case Request Forms or Manage Job Application Forms.

Public Forms manages case-request and job-application intake. Access to a particular form type also depends on its specific management permission.

General settings

  • Form Name: identifies the form.
  • Form Type: Case Request or Job Application.
  • URL Slug: the form's address segment.
  • Active: whether the form is enabled.
  • Public Access: controls the form's public availability.

Branding and instructions

Configure Logo, Organization Display Name, Phone Number, Website, Header Instructions, Primary Color, and Success Message. Public Form Theme can follow the visitor's system/app setting, always use light, or always use dark.

Fields and uploads

In the Fields section, choose which supported fields are visible and required. Hiding a field removes its required setting. Review Max File Size (MB) and Allowed File Types for uploads. Case type availability also depends on Allow on public case request form under Case Types.

Notifications

Configure the submitter's confirmation email subject and body and the staff notification email addresses. Review recipients and wording before making the form available.

Verify the form

  1. Save the form.
  2. Use the copied public URL to inspect the visitor experience.
  3. Check required fields, upload rules, branding, confirmation text, and routing with an approved test submission.

A public intake form allows submissions; it does not give visitors access to internal case records.
