Creating a Public Case Request Form

Manage reusable intake forms in Settings > Public Forms with the required form-management permissions.

Create the form

  1. Open Settings > Public Forms and start a new form.
  2. In General, enter a descriptive Form Name and choose the Case Request form type.
  3. Set a unique URL Slug using lowercase letters, numbers, and single hyphens. Review the generated value before saving.
  4. Configure Active and Public Access for the intended availability.
  5. Review Branding, Fields, and Notifications.
  6. Choose Create Form, then open the saved form using its public-link control to check the requestor experience.

Choose the correct form type

Public Forms also supports job applications. A job application form serves a different intake purpose and does not follow the case request conversion workflow. An existing form's type is fixed so its public link keeps the same submission behavior. Create the correct type at the outset.

Plan the information you need

For example, a general intake form might ask for company and contact details, case type, requested services, instructions, subjects, and documents. Require information your team needs to evaluate the work, and use clear instructions for anything a requestor may not understand.

Creating a form does not create a request. The request is created when someone submits it.
